Benefits for Full-Time Employees

Health Insurance – Preferred Health Systems
Approximately 75% Envision Paid

  • $10 -  Co-payments- All Primary Care Physician Services
  • $10 -  Co-payments for other Outpatient Services
  • $50 / day-  Co-payment – Inpatient Hospital Services-
  • Maximum Annual Co-pay $1000/Individual, $2000/Family 
  • Prescription Drug Program - $15-60 Co-payments/prescription (34 day supply) or $40-150 for Mail Order Prescriptions (90 day supply)
  • Vision Care - One complete eye exam each calendar year & up to a maximum of $150 for eye glasses or contacts per year per member.

Paid leave for up to three days for a death in the employee’s immediate family.
